A suite of templates, forms, fact sheets and checklists have been created to support those looking to set up and run SchoolKit Clinics for children and families in their area. Use these documents, in conjunction with the information provided in the MRID SchoolKit, as tools to help with clinic preparation, facilitation and follow-up so that your clinics can be as effective as possible in meeting the needs of the children and families involved.

Do feel free to utilise these tools however you require in developing documentation for your own school clinics but be aware that their content is copyrighted to the Metro-Regional Intellectual Disability Network and should be acknowledged as such. Please remember that all health or medical research needs to be approved by an ethics committee and survey questionnaires may require permission from your local ethics committee before proceeding.

Most of the documents provided have been developed specifically for the MRID SchoolKit with input from the Kogarah Developmental Assessment Service and Cairnsfoot School. Some of them are based on documents developed with the assistance of the University of New South Wales and the New South Wales Agency for Clinical Innovation. The Summary Clinic Checklist & Information Sheet is based on a similar document created by Lyn Kalkman (ADHC Regional Clinical Nurse Consultant Illawarra Shoalhaven District & Southern District), Jurgen Wille (Social Worker, SESIAHS) and Lif O’Connor (Transition Care Coordinator, GMCT Transition Care Network).
